docs: @langchain/community npm page links to a missing folder

Hi!

Noticed while looking into updating a langchain setup that the npm page for @langchain/community has the "homepage" link pointing at a directory in this repository that doesn't exist anymore.

I've not dug too deep but seems that the community packages now live under a different name(?)

Idea or request for content:

As the npm page is where a lot of people find information on the actual package, I'd suggest updating the README entry there.
